【明報專訊】香港中文大學公共政策研究中心分別於1993年、2004年及2015年進行了本港市民「過去一年曾使用過中醫」的調查。該縱貫性研究結果顯示,本港市民使用中醫的比例,在1993年為29.4%,2004年顯著提升到43.6%,而在2015年只稍微上升到45.2%。換句話說,香港在回歸後中醫服務得到實質性增長,而在2004年至2015年的11年間市民對中醫診療的需求則沒有太大的增長。

An inclusive approach to uphold professional standard—ensuring fair access to the Accredited Registers

According to the government, the Pilot Scheme of the Accredited Registers for Healthcare Professions (AR Scheme) will operate under the principle of “one profession, one professional body, one register”. Members of the accredited professional bodies will receive Certificates of Registration, and they can also use a specified title on their name cards. As such, the public can make informed decisions by looking up the registers of healthcare professionals through the accredited bodies.

Different from doctors, nurses and pharmacists, some healthcare professions such as clinical psychologists are not subject to statutory regulation in Hong Kong. They usually have to rely on a “self-regulation” mode—mostly through voluntary, society-based registration.

醫徹中西:西醫不能說的「身體唔好」

發炎不一定細菌引起

發炎的意思不一定是細菌引起。燒傷、扭傷也會引起發炎。其實很多人在骨痛、胸痛的時候,醫生診斷為關節炎、胸膜炎,都是沒有細菌的發炎。肥胖隨之而有的炎症,可以說是身體狀况不佳的一種表現反應。

最近有一位中年男病人,六年前患上鼻咽癌,當初做了電療和化療,癌症一直沒有復發,只是口乾未能改善。但五年後,聲線開始變差,看醫生後,發覺是左邊聲帶神經半癱,斷定為最近的病變。為何在五年後才有這種病變呢?據說是神經炎或血管發炎所引起。奇怪的是為何在五年後才發生呢?來診時,他的身體完全不在狀態,所以可以說是身體狀况不好的時候,炎症會增加,這病人就是因之前鼻咽癌部位引起這些局部病變。

香港人生活繁忙,尤其是上班一族、需要應付考試的學生或一些與家人朋友的關係出現問題的人,工作、生活環境或學業壓力大,生活節奏快,精神易緊張,加上食無定時,休息不足。容易患上各種腸胃疾病。除了一般較常見如胃痛、胃酸倒流、便秘、泄瀉等病外,近年被診斷患上腸易激綜合症的人亦愈來愈多,估計患病率大概為15-20%。

談腸易激綜合症的防治

腸易激綜合症(Irritable Bowel Syndrome,簡稱IBS)可出現一系列的腸胃問題症狀,可表現為腹痛(或腹部不適)、大便習慣及(或)形態改變、便秘或腹瀉。現代醫學界對本病的成因尚無明確定論,多數認為致病因素與生活或工作壓力、飲食習慣、情緒不穩、女性經期前後、睡眠失調、腸道感染等有關。其實任何人士也有機會患上腸易激綜合症,一般年齡在20-39歲的青壯年及女性患者所佔的比例較多。

陳先生是一名白領,作息定時,偶爾需要加班。近年不斷受背痛困擾,他本以為是坐姿問題,於是矯正坐姿又做瑜伽,但痛症仍一直持續,導致終日鬱鬱不歡。直到痛症加劇,並感到脊椎變硬後他才去求醫,結果確診為強直性脊椎炎(Ankylosing Spondylitis,簡稱AS),同時患有抑鬱症,一個病變兩個病。

Mr Walter Wai-yin Leung is a senior nurse who has been actively participating in various forms of overseas relief missions in Pakistan, Philippines, Nepal, and Liberia since 2010. He has also engaged in long term poverty alleviation efforts, such as carrying out health promotion and disease prevention project within the slum areas of Manila, the Philippines, and looking after the sick and the destitute in their final days in Calcutta, India. In 2017, he was honoured the Hong Kong Humanity Award for his relentless service for the vulnerable.

居住在美國的中國人,病了看西醫,用西藥,常常因為體質和生活習慣不同,出現與本土人士不一樣的治療效果。多年前有一個六十多歲的中國籍病人,來我診室做同位素心肌灌注掃描(Myocardial Perfusion Scan)及心臟核磁共振成像(Cardiac MRI),發現心臟並無大礙,只是心率每分鐘只有三十六次。細問之下,才知他有高血壓,他的心臟專科醫生開給他貝他阻斷劑阿替洛爾(Beta Blocker:Atenolol )。結果血壓是正常了,但心跳卻出奇的緩慢,初時他的醫生認為這只是藥物的副作用,所以將藥量減半,但並無改善,再減到四分之一,仍是老樣子,後來索性停了藥,心跳回復正常,而血壓並未回升。

三高治療與一氧化氮 (尹浩鏐)

以前對於為什麼對藥物的反應因人而異,醫生也不知。我的同窗好友潘建民醫生,是加拿大和美國雙重外科學院院士,他對於這種差異的原因,認為是某一種體內因素產生一種意想不到的特別的血管功能。以前一般心臟內科醫生也不知道是什麼,後來發現是經由血液流動對內膜衝擊摩擦效應而產生的一種當初稱之為微血管內膜細胞素(Endothelin)的東西,現今科學昌明,杜克(Duke)等名校已可測出此一極微量的元素,其實就是令一九八八年諾貝爾生理醫學獎得獎者獲獎的功臣一氧化氮(Nitric oxide,NO)。以前科學家們從未想到這常見而普通的一氧化氮,竟能舒張血管,使缺氧細胞生存;其次更令腎細胞減少因缺氧而產生的血管緊張素(Angiotensin),從而使血壓降低,減低心血管危險指數(The index of Cardiovascular risk)。

Dr Eliza Cheung specialises in disaster mental health and psychological first aid for her clinical practice, field work, and research. She now works as a full-time clinical psychologist at the Red Cross. Over the years, Cheung has also coordinated emergency mental health and psychosocial response and preparedness programmes in Hong Kong, Liberia, Nepal, Bhutan, Singapore, and various provinces of China. In 2015, in recognition of her unremitting contributions to the promotion of mental health in the community, she was honoured as one of the Hong Kong Ten Outstanding Young Persons.

古方洗染頭髮散芬芳

頭髮,又稱「血餘」,意指血之剩餘,氣血充足才能把剩餘營養供給頭髮,所以頭髮的生長依賴血的滋養;頭髮是否富光澤,反映身體氣血虛實狀况。《黃帝內經.素問》中提到「腎者……其華在髮」。頭髮的生長或脫落、潤澤或乾枯,與腎中精氣是否充足有關。因此想養護秀髮,必然從氣血、補腎入手。

古人沒有五花八門的洗髮護髮產品選擇,且洗澡洗髮是一種儀式。沐浴一詞中的「沐」字意指雙手捧水洗髮的樣子。因為洗澡洗髮是古代達官貴人才能享受的特殊待遇,其頻率和步驟都有明確的等級區分,地位愈高的人士沐浴儀式愈複雜。因此歷史中遺留下的美容護髮古方,大都是流傳自皇族、後宮的秘方。

不少人說,足部是人生的第二心臟。我們的健康其實都是靠一雙腳,雙腳健康,行得走得,人也就健康。可惜的是,現代人願意花大量時間金錢來保養丁方幾吋的一張臉,對於一雙腳,則相當忽視。據說人類終其一生或要走上18萬公里的路程,經常行山遠足的或不止此數,所以一雙腳不能不好好保護。出現在腳上的毛病很多,其中最普通的兩種,或者你和我都曾經經歷 ── 就是腳底疣和雞眼,這兩個小毛病很容易發生,擾人而且很容易被混淆。

據統計數字顯示,腰痛佔了半數是由腰椎間盤突出症引起。隨著年齡增長,椎間盤出現退行性變異,遇到急性損傷或慢性勞損,皆可致腰椎間盤纖維環部分或全部破裂,髓核向外膨出或脫出,壓迫和刺激脊髓、神經和周圍組織所引起腰腿痛及活動障礙;嚴重會出現皮膚麻木、肌肉萎縮無力、腰部麻痺、大小便失禁,甚至癱瘓。

腰椎間盤突出症 健身煲劇也高危

別以為患者主要是從事體力勞動者,事實上,因體力勞動而發生腰椎間盤突出症的只佔約15%。近年,病患者不乏年輕及非體力工作人士,他們會因為運動健身姿勢不當、躺在沙發上長期「煲劇」、坐在床上玩手機,或者長期坐在電腦桌前工作等,令腰部長期承受壓力而出現腰椎間盤突出症。

【明報專訊】子宮肌瘤,又稱子宮肉瘤或子宮纖維瘤,在中年女性身上十分常見。絕大部分肌瘤屬良性,但患者會有經血過多,小部分或有經痛等問題。肌瘤太大可引致腹痛或腹部隆起,甚至壓迫膀胱導致尿頻。不是所有子宮瘤都要立即割除,可以透過觀察和藥物緩和徵狀。近年,子宮肌瘤患者有年輕化迹象,愈來愈多青春期少女患病。

【女性健康】禍從口入 提早發育 子宮肌瘤向少女埋手!

高發年齡40至50歲

在香港,4個女性中就有1人患子宮肌瘤,高發年齡為40至50歲,20多歲的女士很少發病;但近年患者有年輕化趨勢,甚至有些未有性經驗的女生患上子宮肌瘤。近年更有醫學報告指出,子宮肌瘤在年幼女孩中出現,外國最新報道指年紀最小患者是9歲,內地報道最年輕患者是15歲,剛過20歲的患者也不罕見。最近看到一則新聞,黑龍江一名26歲的女病人被發現肌瘤遍佈子宮,最大的直徑達9公分,最小的如米粒般大小,大大小小總共50多個,最終通過手術摘除。

Dr Larina Chi-lap Yim is a psychiatrist at Shatin Hospital’s Department of Psychiatry. In 2011, she collaborated with The Salvation Army, St. James’ Settlement and The Christian Concern for the Homeless Association to conduct a study to determine the prevalence of mental illness in the homeless population in Hong Kong. In 2014, Yim decided to take a step further and proposed the “Cure Angels” Community Health Service Spots project to The Salvation Army. Together, they set up the city’s first-ever mobile service station, offering physical and mental health examinations to homeless individuals.

Mental illness among the homeless in Hong Kong: The sorry state of being left outside alone

An elderly man who had been homeless for many years was located by a social worker from an NGO. The man had a history of schizophrenia. After spending much time and effort, the social worker eventually convinced the man to seek treatment at a hospital. Yet, when they arrived at the Accident and Emergency (A&E) Department, the patient changed his mind. The social worker asked for help from A&E nurses and made an emergency call. However, according to the Mental Health Ordinance, the application of sending a patient to a hospital for observation has to be made by either a relative of the patient, a registered medical practitioner or a public officer in the Social Welfare Department. Therefore, the social worker could do nothing but to let go of the patient when he refused hospital admission, considering he had no record of injuring others.

Dr Ning Fan graduated from the University of Hong Kong in 1994, he is a surgeon working in Hospital Authority. Meanwhile, being a field worker of Hong Kong Red Cross and Medecins Sans Frontieres, Dr Fan had been deployed to conflict zones and areas with natural and man‐made disasters. In 2011, Dr Fan cofounded Health In Action, believing that even vulnerable and neglected population in urban setting should enjoy health equity. Dr Fan received the Hong Kong Humanity Award organized by RTHK and Hong Kong Red Cross in 2009.

Health In Action (HIA)

HIA is a humanitarian organization established in 2011 in Hong Kong. Apart from ethnic minorities and ASRs, HIA also focuses on the health inequities faced by local working poor families, who often prioritise generating income to attain household livelihood over pursuing sustainable and healthy body-mind functions. Their work is rooted in the core belief that every human being has the right to health. HIA is a registered charity in Hong Kong and is a member of the Hong Kong Council of Social Service.

Ethnic minorities and ASRs in Hong Kong: How to achieve equal health care for all?
